Lyra's IFSP/Early Intervention Service

    Lyra's IFSP/Early Intervention Service

     I posted Lyra's IFSP on our website.  She was 4 months old when it was written. She is now 15 months and we are just starting her annual review.  It's a PDF file, so hopefully you can open it.  I know there have been a lot of questions on here about EI services and I just wanted you all to be able to see an example of an Individualized Family Service Plan (IFSP).  In case anyone is unfamiliar with the term, an IFSP is basically an IEP for kids who are not yet school age, so approximately birth to 4 years old.

    Did you`also get a vision assessment from a Vision specialist,  If you didnt I would be asking for a Vision specialist to work with her. In some places that is thru the school district and in other  another agency.

    IFSP are Individual Family Service plans and they are for children 0 to 3 in most places.

    I Looked at the what Lyra see from the confrence and that is an amazingly well done presentation. Thanks for sharing it.

    Yes, Lyra had a functional vision assessment before they wrote the IFSP.  She has had both an OT and a TVI coming to our house once a week for the last year.  I will be posting her assessment on the website sometime this week.

    we have never had a vision therapist, and I have asked our OT a couple of times in the last year, if we SHOULD have one... and they act like it isnt neccesary.  It seems like b/c he has a vision impairment, that a VIT would be what he would need.  IM going to ask her about that at our next appt, and tell her that alot of you guys have a VIT, as well as an OT.

    Where do you get a functional vision assessment done and how old was Lyra when her's was done?  I really think that would benefit Ryker.  We were told there are no vision teachers in SD so I'm not sure where to turn.  He is only getting OT once a month or so to make sure he is on track but I wonder if we shouldn't push the issue more now that he is older and needs to be doing more.  I'm just worried that he's not getting all he should be getting.

     Hi Kelli,

    I added Lyra's functional vision assessment to the website.  It was completed when she was 3 months old, prior to them writing the IFSP.  A social worker and an OT from the Children's Center for the Visually Impaired here in Kansas City did the assessment. Lyra's TVI also works for CCVI, but is contracted by Wyandotte county.  I can ask her who you would need to contact in SD.
    I have one more thing to add to the site later today, Lyra's "Present Level of Development" information.  It was also completed prior to writing the IFSP, when she was around 3 months old and is similar to the functional vision assessment, but includes other areas of development like motor and cognitive.  It was completed by Wyandotte County, not CCVI.

    Mashawna,

    I read Lyra's funtional vision assessment and present level of development.  There are some similar things noted in Ryker's IFSP but Lyra's is much more thorough and I can tell their main focus is her vision.  I think I'm going to email our consultant with the SD School for the Blind and Visually Impaired and see if they can do a functional vision assessment.  I would think they could or do only TVI's do that?  I really think that it would help his OT understand how Ryker sees and help him use his vision more even if we can't get a TVI.

    Thanks for allowing us to look at Lyra's plans, I'm sure it has helped a lot of people and it's good to compare and give examples to our own teachers.
